1. How It Works: No purchase or payment of any money is necessary to enter. There is no cash or exchange value for prizes, and prizes are not transferable. All federal, state and/or local rules and regulations apply. Void where prohibited by law. The Nowak Aesthetics "A New You" contest runs from 10/15/07 through 02/11/08.
2. Eligibility: Must be 18 or older to nominate. The contest is open to all legal U.S. residents of San Diego County. Employees of KGTV, McGraw-Hill Inc. and Internet Broadcasting Systems, its parents, subsidiaries, and affiliates, all sponsors, advertising and promotion agencies and all others associated with the development and execution of this contest and their respective employees, families, and assigns are not eligible to enter. Limit one winner per household.
3. To enter: Contestants may enter on-line at 10news.com by filling in the required fields in the "A New You" section presented by Nowak Aestherics; or by mailing your entry to KGTV, c/o KGTV 4600 Air Way San Diego, CA. 92102. Mail entries should include: your name, address, phone number. All entries must be received by 12:00 midnight Pacific Time on Feb 01, 2008. You may enter more than once. Mail in entries must be postmarked individually. On line entries must be limited to one entry per day, and no mechanically produced entries will be allowed. One winner from the dk Hair transform your look contest will be selected each month. All winners are final.
4. Prizes: One winning entry will be selected each month the contest is running to receive a free Botox treatment(valued at $350) OR the winner has the option of not taking the Botox treatment in exchange for $100 credit towards another service or procedure. Acceptance prize offered constitutes permission to use the winner's name and/or likeness for advertising for advertising / trade purposes without further compensation, unless prohibited by law. Advertiser has the right to the discretion on use of services provided.
5. Selection of winners: The winner will be selected at random on 10/31/07, 01/31/08, and 02/28/08 from all entries submitted to KGTV from mail-in and web entries. Winner will be notified by mail or phone and must identify themselves with a valid driver's license or state identification card and complete and sign an affidavit of eligibility, release and indemnification in the time period specified by the station to claim the prize. Prizes not claimed by Dec 31st, 2008 will be forfeited. No substitution or transfer of prize will be permitted. Unclaimed prizes will not be awarded. Payment of all taxes and fees, and procurement of all licenses is solely the responsibility of the Prize Winner.
6. Conditions: The contestants and winners release the owners and employees of KGTV and McGraw-Hill Inc., its parents, subsidiaries, and affiliates, Internet Broadcasting Systems, Inc. (IBS) all sponsors, advertising and promotion agencies and all others associated with the development and execution of this contest and their respective employees, families, and assigns, from and against any and all liability with respect to or in any way arising from the contest and the awarding and use of the prizes, including liability for personal injury and/or property damage. By entering, unless prohibited by law, the winners consent to use of their names, photographs and/or likeness for advertising or publicity purposes for this and similar promotions without compensation. KGTV and 10news.com retain the discretion to make decisions regarding the interpretations and applications of these rules. All decisions of KGTV and 10news.com are final. KGTV, 10news.com and IBS are not responsible for any computer and/or email malfunctions, failure of any email provider service or any other impediment to delivery of entries.
7. Responsibility & Liability: The sponsors, KGTV, McGraw-Hill, 10news.com and IBS assume no responsibility or liability for damages, losses or injuries resulting from the use of or acceptance of any prize. The sponsor reserves the right, on its sole and absolute discretion to interpret compliance with the letter and intent of these rules, and to disqualify any applicant deemed to have violated these rules or to have filled out any application incorrectly at anytime prior to the distribution of prizes. Additions or deletions to the rules are subject to change without notice. In the event the contest is postponed for any reason, the timetable may be altered at the discretion of KGTV.
8. Odds of Winning: The odds of winning depend upon the number of people entering.
